Credit and/or purchase card issuers

Registration, update or cancellation form *

To register, the following PDF files must be uploaded:

  • Formulas II, III and IV (each one must be fully integrated, with the signature and clarification of the person responsible for the Information Regime and the legal representative/attorney-in-fact)
  • Articles of incorporation or bylaws and their modifications
  • Annual balance sheet with report of the external auditor and certification of the corresponding professional council
  • Copy of the credit and/or purchase card(s) issued, if applicable.

Important

Once the upload has been made, you will receive an email indicating that you will have 30 (thirty) calendar days to submit a note at the Ticket Desk, signed by the legal representative or authorized person (with a legalized copy) addressed to the Authorization Management of the Superintendence of Financial and Exchange Entities.
The note will revalidate the registration and state – as an affidavit – that “all the documentation presented in the application is a true copy and that the originals are available to this Superintendence of Financial and Exchange Entities”, indicating the address.
